ALS Arizona was formerly named Equipment Loan Closet with the ALS Association AZ Chapter. They are currently ALS Association.
* They not have the inventory to take donations from the public at this time* Only accepting Electric Hoyer Lifts and the 2 of theTobii devices: 13-inch (TD I-13) and the 16-inch (TD I-16) sizes at this.

Eligibility
A person must have a diagnosis of ALS. Also need to have a diagnosis from a Neurologist.

Acceptance Criteria
Currently only taking ALS donations from the families of our ALS clients. Do not have the inventory to take donations from the public at this time.

Yes. Need to have a diagnosis from a Neurologist for medical equipment.
