FSL is now AllThrive365
AllThrive365 does not have the ability to pick up, transport, or store donated beds. Because of this, they maintain a donation list and when someone is in need of a hospital bed, a Patient Care Advocate can help coordinate directly between the donor and the client or family for pickup and transfer. Eligibility
18+ or older, have AHCCCS/Medicaid, meet income guidelines for housing/energy and those with SMI.

They service patients throughout Maricopa County.
Acceptance Criteria
Assist those that were recently discharged from a Dignity Health Hospital within 90 days, those that are high-risk for re-hospitalization.
